We welcome you as a new patient to Southern Tennessee Cardiology and Vein Center. To help ensure your first visit is smooth and worry-free, we have provided some useful information and recommendations below.

Here is a list of items you will want to bring along:

• Insurance card(s)
• Photo identification, such as a driver's license
• Referral form (for HMO insurance, if required)
• List of current medications you are taking including prescriptions, over-the-counter medications and any herbs (do not hesitate to just bring in the bottles if you are unsure).
• Information about your medical and surgical history. Bring any pertinent x-rays or records you may have.
• Important questions you would like to ask the doctor.

Having these items ready will help us serve you more efficiently and ensure that all of your concerns are addressed at your appointment.

We ask that you plan on being in our office for one to two hours. During your first visit, the doctor will thoroughly review and discuss your medical history.

The doctor will likely perform a physical examination and depending on the findings, your symptoms or your medical history, the doctor may want to discuss options for various tests and treatment plans.Never hesitate to ask the doctor or physician extender any question you should have. We want to help you understand your health status so that we can work together to maximize your quality of life.

The following forms can be downloaded to your computer and printed at home. Alternatively you may create an account via the patient portal and complete these forms securely online. By filling out these forms ahead of time you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will be happy to assist you with your questions on the day of your visit.
