Here is a list of items you will want to bring along:
- Insurance card(s)
- Photo identification, such as a driver's license
- Referral form (for HMO insurance, if required)
- List of current medications you are taking including prescriptions, over-the-counter medications and any herbs (do not hesitate to just bring in the bottles if you are unsure).
- Information about your medical and surgical history. Bring any pertinent x-rays or records you may have.
- Important questions you would like to ask the doctor.
Having these items ready will help us serve you more efficiently and ensure that all of your concerns are addressed at your appointment.
YOUR INITIAL APPOINTMENT WILL LAST FOR 1-2 HOURS. DURING YOUR FIRST VISIT, THE DOCTOR WILL THOROUGHLY REVIEW AND DISCUSS YOUR MEDICAL HISTORY, SYMPTOMS AND ANSWER ANY QUESTIONS YOU MAY HAVE.
The doctor will likely perform a physical examination and depending on the findings, your symptoms or your medical history, the doctor may want to discuss options for various tests and treatment plans.Never hesitate to ask the doctor or physician extender any question you should have. We want to help you understand your health status so that we can work together to maximize your quality of life.
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will be happy to assist you with your questions on the day of your visit.
INSURANCE
Dr. Basaraba is an in-network provider for most major insurance plans. We are happy to assist you by billing your insurance company. Please bring your current insurance card(s) to each appointment so that we may verify your billing information. Co-pays are due at the time of service.
CANCELATION POLICY
YOUR APPOINTMENT TIME IS RESERVED EXCLUSIVELY FOR YOU.
If you are unable to keep your appointment, kindly call us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another person. In the event you cancel within 24 hours of your appointment, we will charge a cancellation fee of
$25 for Office Visits
$50 for Testing
$150 for Nuclear Stress Tests.
Kindly note the higher fee for office procedures. Testing slots are especially difficult to fill on short notice because of required insurance authorizations and regulatory requirements. Additionally, nuclear stress tests require us to purchase a medication specifically for you and your designated appointment time. If we are unable to cancel this medication order, the cost will be billed directly to the patient and is not billable to insurance.
